Inspectors found: Staff did not always assess risks to people's health and safety or mitigate them where identified.

Due to understaffing, staff were working long hours over multiple days, sometimes without breaks which risked fatigue and poor care as a result. Staff told us they understood how to recognise the signs and symptoms of abuse, but inspectors found not all unexplained injuries had been reported. Leaders had created an institutionalised service, where care was provided according to what was convenient for staff rather than what people preferred.

Relatives told us that management could be unhelpful and defensive when they raised issues. Inspectors also saw some people being fed food that was a known risk to them.
